How Smiles for a Lifetime Works

Smiles for a Lifetime operates much like a traditional dental office, but our office is located on school property and follows the same school calendar as the Clarendon County School District. We work closely with school staff to schedule appointments that reduces missed classroom time. Students receive care from licensed dental professionals during the school year, and transportation to and from appointments is available if needed. To participate, a parent or legal guardian simply completes a consent form.

A completed consent form is required each school year for students to receive care through Smiles for a Lifetime.

You may fill out the form online or download a paper version. Completed paper forms can be emailed, mailed, or returned to your child’s school nurse.
